Importance of Disconnected Pancreatic Duct Syndrome in Recurrence of Pancreatic Fluid Collections Initially Drained Using Lumen-Apposing Metal Stents

چکیده

Background & AimsIndwelling plastic endoprosthesis in patients with disconnected pancreatic duct syndrome (DPDS) reduces the risk of fluid collection (PFC) recurrence. Although lumen-apposing metal stents (LAMS) are used increasing frequency for PFC drainage, they require timely removal and little is known about their effects DPDS recurrence PFC.MethodsWe performed a prospective study who underwent endoscopic ultrasound-guided drainage using LAMS were found to have DPDS. After resolution PFC, replaced double-pigtail stents. The primary outcome was compare between did vs not receive replacements after LAMS.ResultsOf 188 treated LAMS, 94 had DPDS, 71 intact ducts, patency unknown 23. In replacement successful 70 (74.5%) technically unsuccessful 24 (25.5%). Lumen apposing metal stents for pancreatic fluid collections: Recognition and management of complications

For patients recovering from acute pancreatitis, the development of a pancreatic fluid collection (PFC) predicts a more complex course of recovery, and introduces difficult management decisions with regard to when, whether, and how the collection should be drained. Most PFCs resolve spontaneously and drainage is indicated only in pseudocysts and walled-off pancreatic necrosis when the collectio...

Pancreatic Pseudocyst Drainage Using Lumen-Apposing Metal Stents.

MK Currently, the most common approach is with endoscopic ultrasound (EUS) guidance. Using EUS with a large working channel, the endoscopist can puncture and then drain the fluid collection after checking local vasculature with Doppler ultrasound. The drainage will depend on the type of fluid collection. For example, if the patient has a simple pseudocyst, the endoscopist could place a single 1...

Safety and efficacy of endoscopic ultrasound-guided drainage of pancreatic fluid collections with lumen-apposing covered self-expanding metal stents.

BACKGROUND & AIMS Plastic stents, placed via endoscopy to drain pancreatic fluid collections (PFCs), require repeat access. Covered metal stents are larger in diameter and can be inserted in a single step, but can migrate. We evaluated the safety and efficacy of a lumen-apposing, covered, self-expanding metal stent (LACSEMS) for PFC drainage. METHODS We performed a prospective study of the ou...

Disconnected pancreatic duct syndrome.

Disconnected pancreatic duct syndrome (DPDS) is characterised by disruption of the main pancreatic duct with a loss of continuity between the pancreatic duct and the gastrointestinal tract caused by ductal necrosis after severe acute necrotising pancreatitis treated medically, by percutaneous drainage, or necrosectomy. There are no clear epidemiological data on the real incidence of DPDS; appro...

Lumen-apposing metal stents (LAMS) for pancreatic fluid collection (PFC) drainage: may not be business as usual

MESSAGE Lumen-apposing metal stents (LAMS) have been recently developed to improve treatment outcomes in the endoscopic management of pancreatic fluid collections (PFC), particularly in walled-off necrosis (WON), to facilitate better drainage of necrotic contents and minimise the risk of perforation and peritoneal leakage. In an ongoing randomised trial, we observed serious adverse events that ...
